Zoom Meeting Request Form for District Committee Chairs

To ensure District Meetings are scheduled with proper authorization and to avoid schedule conflicts, District Committee Chairs are requested to use the following form to book a Zoom meeting time. To avoid cancelation of meetings, please use this request form to book all meetings requiring the District Zoom account.

Please provide at least 7 days notice for requests.

The meeting link will be provided to the Committee Chair noted in the request form.
